Appointments
Appointments may be scheduled by calling our centralized scheduling number at 713.425.8100 or 866.MRI.XRAY.
You will be directed to the appropriate office for your examination. For your convenience, we provide bilingual staff at all of our locations.
You may download a patient information form and fill it out in advance of your appointment. The completed form can be sent by e-mail to scheduling@drhmri.com or faxed to 713.425.8182 which will expedite your sign in process the day of your examination.
If you prefer, you can bring the completed form with you and give it to the receptionist on the day of your examination.
